Experienced in Complex Care

Patients will receive high quality care from Registered Nurses with hospital experience in ICU and ICU Step-Down units. Our nurses bring strong assessment skills and advanced clinical judgement developed through caring for complex patients. Their expertise allows them to identify early signs and symptoms of illness, intervene promptly, and help keep patients stable - reducing the likelihood of emergency room visits.

Patients will receive complete and thorough care from experienced registered nurses who are fluent in English, Spanish, and Vietnamese.

Patients will receive care from nurses who are fellow Santa Clara County community members who understand first-hand how H.R. 1 is impacting our communities.

Patients will receive high-quality care from physicians with expertise across multiple specialties, including GYN/Oncology, Cardiology, Endocrinology, Asthma/Allergy, and Medication-Assisted Treatment (MAT) for substance use. Their expertise enables them to identify early signs and symptoms of illness, intervene promptly, and help keep patients stable - reducing the likelihood of emergency room visits.

Patients will receive complete and thorough care from experienced physicians who are fluent in English, Spanish, and Vietnamese.
